SOLACE Ohio Statewide Network Registry

1) SOLACE Chapter Registration Form - This registration is for groups that plan to have the SOLACE name and logo for their chapter.  If you have an established support/advocacy group that is a SOLACE chapter, please use this registration form.

2) SOLACE Ohio Friends Registration Form - This registration is for any support/advocacy group that would like the benefits of being a SOLACE chapter, but would like to maintain their individual group name and logo already in use.

3)   SOLACE SIBS Chapter Registration Form - This registration is for anyone interested in registering a SOLACE SIBS (Surviving Our Loss And Continuing Every Day - Sharing Insight as Brothers and Sisters) youth group in their area. SOLACE SIBS is for high school and college aged youth (up to 24 years of age) that have been touched by the loss of a friend or a loved one to prescription drug abuse.

SOLACE SIBS is not limited to blood-related siblings, because there are numerous young people that have lost best friends, cousins, aunts, uncles, etc., and are having a difficult time processing that loss.  This support/advocacy group is for young adults that are interested in supporting one another through this difficult time, and to promote their own advocacy efforts to end the tragedy of prescription drug abuse in Ohio.  If you would like to register a SOLACE SIBS group, please use this registration form.

Some of the benefits of registering in the SOLACE Ohio Network include:
